**Job Title:** Care Coordination Specialist

Fairbanks Resource Agency Inc is seeking a highly organized and detail-oriented individual to fill the role of a Care Coordination Specialist. This is an exciting opportunity to work with individuals with disabilities and help them achieve their goals.

Job Description:

The Care Coordination Specialist will assist in the planning and implementing of Plans of Care (POCs), ensure the well-being and safety of program participants, provide guidance and training that facilitates independence, recognize and respect the rights of the participant, and maintain accurate records and reports.

Responsibilities:

  • Assist in the planning and implementing of Plans of Care (POCs).

Requirements:

  • Demonstrate a personal commitment to the development and implementation of the least restrictive residential services for participants.
  • Demonstrated skill in implementing and documenting direct participant training.
  • Flexible scheduling is necessary.
  • Must have reliable transportation and maintain a good driving record.
  • Must possess the knowledge and ability to utilize a computer and electronic medical record (EMR) system.
