Job Overview

We are seeking a skilled Nurse Care Coordinator to join our team at Foundation Health Services.

About the Role

This is a full-time position that requires providing high-quality nursing care to patients in a non-acute environment. The successful candidate will assess, plan, implement, and evaluate patient care according to organizational policies and professional standards.

Key Responsibilities
  • Patient Assessment: Conduct thorough assessments of patients' needs, review medical histories, and complete assessments as per policy.
  • Care Planning: Develop personalized care plans, including discharge planning, based on patient-specific needs.
  • Care Implementation: Deliver direct patient care, coordinate with healthcare teams, and supervise staff as needed.
  • Evaluation and Revision: Continuously evaluate patient response to care and revise plans as necessary.
  • Documentation: Maintain accurate records of assessment, planning, implementation, and evaluation.
Qualifications and Requirements
  • A valid RN license in the state of practice.
  • BLS certification.
  • Desirable: additional certifications or continuing education, experience in the clinical area, and maintenance of clinical performance competencies.
Estimated Salary

$65,000 - $85,000 per annum, depending on experience.
