Job Summary:

Benchmark at Rye is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) / Registered Nurse (RN) to join our team of healthcare professionals. As a key member of our care team, you will be responsible for delivering high-quality nursing care to our residents in a warm and comfortable environment.

Key Responsibilities:

  • Assessment and Communication: Accurately assess, communicate, and document residents' status, including any significant changes in behavior and health.
  • Reporting and Documentation: Report any notable changes in resident behavior and health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director as required by state regulations.
  • Leadership and Supervision: Direct and supervise the daily work assignments of Resident Care Assistants and closely mentor, guide, and direct Lead Resident Care Assistants in all aspects of their job.
  • Care Coordination: Coordinate care needs with community providers through an effective case management process.
  • Service Delivery: Effectively balance service demands with supporting resident independence.
  • Prevention and Response: Recognize and assist in the prevention of elder abuse, neglect, and exploitation, and report to appropriate sources as required by state regulations.
  • Emergency Response: Respond to emergencies and personal emergency response system, and know when to call for backup.

Requirements:

  • Licensure and Certification: Current state license as an LPN / RN and CPR certification.
  • Education and Experience: Graduate of an approved LPN / RN program (per state requirements) and a minimum of 1 to 2 years of experience working as a nurse, preferably with experience working with the elderly in a paid or volunteer position.
