Job Summary:We are looking for a skilled and experienced CNA to provide personal care and treatment to patients in their homes. The ideal candidate will have a strong commitment to delivering exceptional patient care and a passion for working in a dynamic and supportive team environment.
Responsibilities:- Provide personal care and treatment to patients in their homes
- Plan and prepare meals, assist with feeding, and perform range of motion and simple procedures as an extensional therapy service
- Take and record temperatures, pulse, respiration, and blood pressure as ordered
- Assist with laundry and address patient safety needs
- Report on patient's condition to the assigned nurse and participate in regular Interdisciplinary Group (IDG) Case Conference meetings
- CNA licensure in Oregon
- 1+ years of experience preferred
- Ability to work independently and demonstrate critical thinking
- Current BLS certification from AHA
- Be a licensed driver with access to a reliable vehicle
