- Direct physical therapy treatment and evaluate home environment.
- Assess and evaluate therapeutic, rehabilitative, and functional status for the development of the total plan of care.
- Provide patient, caregiver, and family education and counseling.
As a Physical Therapist at Connected Home Health & Hospice, you'll be a vital part of our team, delivering compassionate and personalized care to our clients. We're looking for a motivated and dedicated individual who shares our passion for changing lives.
Requirements:- Active Physical Therapist (PT) licensure in Oregon.
- 2+ years of Physical Therapy experience.
- Critical thinking skills and ability to work independently.
- Hold a current BLS certification from the American Heart Association.
