Position Summary:

Become part of a pioneering approach aimed at reforming the child welfare system by integrating substance use treatment within the home environment. This strategy allows children to remain with their families while parents focus on recovery, rather than facing separation. The role involves providing individualized home-based support and community group activities centered on parenting and recovery skill development, tailored to each client's case plan. Services will promote healthy and effective parenting practices, including strategies for managing mental health, connecting families with community resources, and offering educational support in parenting skills. The specialist will maintain a caseload of 12 families, ensuring timely and effective case management, and will collaborate with the In-Home Recovery team to coordinate comprehensive services that address both individual and family objectives. Accurate and timely documentation of client interactions is essential, requiring meticulous attention to detail and adherence to contract compliance and case noting standards. Participation in clinical supervision is expected, with a commitment to fostering respect and trust within the community, thereby embodying the organization's values and mission.


Key Responsibilities:

Case Management (60% - 70%)

  • Engage in the Yale Family Based Recovery Program training model, ensuring adherence to its principles with complete fidelity.
  • Facilitate home-based coaching and ongoing support that respects family dynamics through a trauma-informed approach, enhancing client satisfaction and retention.
  • Manage a caseload of 12-14 families, providing timely assistance and follow-up on all case-related needs.
  • Offer community and internal referrals to families as necessary.
  • Ensure client privacy by completing all necessary releases of information before contacting external stakeholders.
  • Document all client interactions and support within 24-48 hours, including communications with external stakeholders such as social workers and case managers.
  • Maintain organized and up-to-date client documentation, including files and relevant paperwork.

Group Management (10% - 20%)

  • Conduct weekly support groups for parents seeking to develop new parenting and stress management skills, contributing to improved behavioral health.
  • Provide insights to group participants that foster individual confidence and engagement.
  • Organize and facilitate a weekly social club for families.
  • Document a summary of group activities post-social club, including progress towards goals and individual observations.
  • Assist with managing incentives related to the social club activities.

Operational Duties (10% - 20%)

  • Timely entry of case notes, assessments, and all client-related forms to ensure accurate billing and service delivery.
  • Respond to internal inquiries for information within 24-48 hours to support colleagues' workflow.
  • Complete all required billing and reporting tasks by specified deadlines.

Qualifications:

  • Bachelor's degree in family social services, social work, behavioral health/psychology, or a related field.
  • 2-3 years of experience working with families facing significant barriers and/or involvement with child welfare systems.
  • 2-3 years of experience addressing behavioral health needs with individuals and families.
  • Fluency in Spanish, Somali, Hmong, and/or Karen is preferred but not mandatory.
  • Strong written communication skills, with the ability to convey updates and inquiries clearly via email, and proficient in editing for clarity and correctness.
  • Effective calendar management skills to maintain an organized record of work-related activities.
  • Proven ability to work collaboratively within a team, contributing to a positive work culture and addressing conflicts professionally.
  • Excellent oral communication skills, demonstrating clarity and persuasiveness, with active listening abilities.
