Position Overview:
Advocate Aurora Health is on the lookout for a dedicated Clinical Nurse Specialist (CNS) in Transplant Care to join our esteemed nursing team.
Key Responsibilities:
As a Clinical Nurse Specialist, you will be responsible for:
- Providing and coordinating comprehensive patient care through the nursing process, ensuring safe and therapeutic care in line with established standards and protocols.
- Utilizing evidence-based practices and clinical judgment to foster the safest patient environment and enhance the overall patient experience.
- Employing the nursing process to assess patient needs, develop tailored intervention plans, and evaluate their effectiveness.
- Engaging patients and their families in care planning and implementation, maintaining a therapeutic relationship throughout the healthcare journey.
- Monitoring significant changes in patient conditions and proactively adjusting care plans as necessary, collaborating with the healthcare team to ensure patient-centered care.
- Implementing strategies aimed at minimizing patient risks and enhancing safety, while assessing readiness for learning and adapting approaches accordingly.
- Coordinating discharge plans through an interdisciplinary approach, applying evidence-based practices in patient care delivery.
- Evaluating patient, team, and unit outcomes consistently, taking appropriate actions when necessary.
- Participating in shared governance initiatives to enhance processes and patient outcomes.
- Demonstrating effective communication, feedback, and conflict resolution skills, while promoting collaboration among healthcare team members.
- Assuming a leadership role within the care team and delegating tasks appropriately to unlicensed personnel.
- Fostering a culture of safety by identifying and addressing potential threats to patient safety.
- Engaging in quality improvement initiatives to enhance departmental functions and the patient care experience.
- Contributing to personal and professional development through participation in educational programs and meetings.
Scheduled Hours:
This is a full-time position on the night shift, requiring availability every third weekend and 50% of holidays.
Licenses & Certifications:
Applicants must possess a Registered Nurse license issued by the state of practice and Basic Life Support (BLS) certification from the American Heart Association.
Qualifications:
Completion of an accredited nursing program is required. New graduates are encouraged to apply.
