Care Coordinator Specialist

We are seeking a highly skilled Care Coordinator Specialist to join our team at MD CLINICS. As a key member of our staff, you will be responsible for providing exceptional care coordination and case management services to our patients.
About the RoleThis is a full-time position that requires a bachelor's degree in a related human service or healthcare field from an accredited college or university. You will need to have a valid Kentucky driver's license and pass a background check. We prefer candidates with a minimum of two years' experience working with homeless/low-income populations and persons with behavioral health care disorders.
A successful candidate will possess strong communication and interpersonal skills, as well as the ability to work effectively with patients, families, and community resources. You will be responsible for linking patients to necessary healthcare services, assisting with paperwork, and educating patients on available resources.
Key Responsibilities- Provide care coordination and linkages with services such as transportation, prescription assistance, eligibility assistance, translation, and other identified needs to improve access to and utilization of care.
- Assist patients with completing required paperwork and explain clinic policies to help patients understand and navigate available services.
- Assist uninsured patients with identifying and determining a healthcare payor source (e.g., insurance, Medicaid, Medicare, VA or other public benefits) as eligible and/or access to community-based resources (e.g., medication assistance programs).
- Utilize trauma-informed and evidence-based approaches to assist with service delivery to patients and their families.
- Maintain records and reports using integrated health records which can be audited for accountability.
- Establish and maintain an outside network of applicable community resources, contacts, and relationships as seen fit for the Care Coordinator Specialist.
- Bachelor's degree in a related human service or healthcare field from an accredited college or university.
- Valid Kentucky driver's license.
- Pass background check.
