We are seeking a skilled and compassionate Nurse Practitioner to join our team in providing comprehensive health assessments to at-risk individuals in their homes.

Job DescriptionThis PRN/Part-Time position involves conducting adult and geriatric assessments, medical history reviews, diagnosis, treatment, health education, and documentation according to Matrix guidelines. You will work collaboratively with physicians, case managers, social workers, family members, and other healthcare professionals to provide comprehensive care.
The successful candidate will have a Master's degree, current RN and NP licensure, and board certification in a Matrix-approved specialty. A minimum of 1 year of experience as a Nurse Practitioner, strong computer skills, and excellent communication skills are also required.
Key Responsibilities- Conduct comprehensive health assessments in various settings, including home visits, skilled facilities, and telehealth.
- Develop and implement individualized care plans to address patient needs.
- Collaborate with multidisciplinary teams to ensure seamless care coordination.
- Maintain accurate and up-to-date medical records.
This role offers a competitive compensation package, including $90.00 per completed assessment. As a PRN/Part-Time provider, you will enjoy flexible scheduling and mileage reimbursement. We offer a 401(k) plan and limited voluntary benefits.
Travel RequirementsYou must be able to travel, hold a valid driver's license, and possess adequate automobile insurance coverage. Regular travel to patients' locations is required.
