As a Care Navigator at Centene Corporation, you will play a critical role in developing, assessing, and coordinating care management activities that prioritize member needs and deliver quality, cost-effective healthcare outcomes. With a focus on personalized care planning, you will educate members and their families/caregivers on services and benefit options, advocating for high-quality care through coordination and support.

About the Role:

This position involves evaluating member needs, barriers to care, and resources available to recommend and facilitate the best outcome. You will develop or contribute to ongoing care plans/service plans, identifying providers, specialists, and community resources needed for care. Your expertise will be valuable in providing psychosocial and resource support to members/caregivers and care managers to access local resources or services.

Responsibilities:

  • Evaluate member needs, barriers to care, and resources available to recommend and facilitate the best outcome
  • Develop or contribute to ongoing care plans/service plans, identifying providers, specialists, and community resources needed for care
  • Provide psychosocial and resource support to members/caregivers and care managers to access local resources or services
  • Coordinate between members/families/caregivers and care provider teams to ensure accessible care or services
  • Monitor progress towards care plans/service plans goals and collaborate with healthcare providers for revision or addressing identified member needs
  • Collect, document, and maintain member information and care management activities for compliance with current state, federal, and third-party payer regulators

Requirements and Benefits:

  • Bachelor's degree required; Graduate from an Accredited School of Nursing if holding clinical licensure
  • 2-4 years of related experience
  • Current states clinical license preferred
  • Competitive pay: $22.07 - $37.25 per hour
