Join our team as a Hospice Outreach and Education Specialist, where you will play a key role in promoting hospice programs and services to patients, families, and healthcare professionals.

Key Responsibilities:
  • Develop and implement outreach strategies to increase awareness and understanding of hospice services and programs among healthcare providers, patients, and families.
  • Design and deliver educational programs, workshops, and presentations on various topics related to hospice care, including symptom management, pain control, and end-of-life issues.
  • Collaborate with healthcare teams to integrate hospice services into existing care pathways and ensure seamless transitions.
  • Conduct site visits, attend IDT meetings, and participate in quality improvement initiatives to enhance patient care and outcomes.
  • Provide individualized education and support to patients and families, addressing their unique needs and concerns.
  • Document patient interactions, educational sessions, and any outcomes or concerns in a clear and concise manner, adhering to regulatory requirements and hospice policies.
  • Stay current with industry developments, best practices, and regulatory updates to enhance knowledge and skills.

Estimated salary range: $70,000 - $90,000 per year.
