About the Role

We're seeking a skilled Speech Language Pathologist to join our team on a per diem basis. As a Home Health Speech Language Pathologist, you'll work with patients in their homes, providing evaluation, treatment, and education to help them achieve their goals.

Responsibilities
  • Evaluate patients' communication and swallowing disorders, developing personalized treatment plans to address their needs.
  • Provide speech and language therapy to patients, using evidence-based practices and techniques.
  • Collaborate with our interdisciplinary team to ensure comprehensive care and coordination.
  • Document patient progress and outcomes, providing regular updates to our care team.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Master's degree in Speech-Language Pathology or related field.
  • Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A).
  • Minimum of six months experience as a Speech-Language Pathologist.
  • Home health experience preferred.
  • Current CPR certification.
  • Good organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
