About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team at CenterWell Home Health. As a Speech Language Pathologist, you will play a vital role in providing high-quality patient care and rehabilitation services in the comfort of patients' homes.

Key Responsibilities
  • Evaluate and treat patients with speech, language, and swallowing disorders
  • Develop and implement individualized treatment plans to meet patients' unique needs
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Provide education and training to patients and their families on communication strategies and techniques
  • Document patient progress and maintain accurate records
Requirements
  • Master's degree in Speech-Language Pathology or related field
  • Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A)
  • Minimum of six months experience as a Speech-Language Pathologist
  • Home Health experience preferred
  • Current CPR certification
  • Good organizational and communication skills
  • Valid driver's license and reliable transportation
