Palliative Care Nurse Practitioner Opportunity

About the Role:

We are seeking a highly skilled and compassionate Palliative Care Nurse Practitioner to join our team at Boston Medical Center. As a key member of our clinical team, you will play a vital role in providing high-quality, patient-centered care to individuals with serious illnesses.

Responsibilities:

  • Provide comprehensive palliative care to patients and their families, including symptom management, goal-of-care discussions, and support for end-of-life care.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Conduct thorough assessments and develop treatment plans to address patients' physical, emotional, and spiritual needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance palliative care services and outcomes.

Requirements:

  • Active MA NP license
  • Minimum of 2 years of palliative care experience
  • Strong communication and teamwork skills
