Position Overview

The Senior Care Coordinator is responsible for delivering tailored and medically essential case management services for clients, as well as skills training directly to individuals.

Case Management encompasses services that assist clients in accessing necessary medical, social, educational, and other resources. This includes direct support in obtaining services, care coordination, comprehensive case oversight, and connecting clients to suitable resources. The Case Management process includes:

  1. Evaluating the eligible client to identify service requirements;
  2. Creating a personalized integrated care plan;
  3. Facilitating referrals and related activities to help clients secure necessary services;
  4. Monitoring and follow-up;
  5. Assessing outcomes.

It is important to note that Case Management does not involve the direct provision of medical, clinical, or other direct services. The focus is on managing the case on behalf of the client, rather than providing services directly to the client.

The Senior Care Coordinator will also be tasked with delivering Skills Training and Development services to clients. These services include individual, group, or family face-to-face interactions aimed at developing essential skills (such as self-care, daily life management, or problem-solving abilities) to overcome psychosocial barriers identified by HCI therapists, case managers, and supervisory personnel. Skills development is facilitated through structured interventions aimed at achieving goals outlined in the personalized integrated care plan, along with monitoring progress in skill acquisition. Skills Training and Development includes the direct provision of rehabilitative services but does not cover medical or clinical services.

The Senior Care Coordinator:

  • Utilizes discretion and judgment to analyze, interpret, and deduce necessary actions based on the unique facts and circumstances of each case.
  • Operates independently without daily supervision, assessing potential actions and making decisions when supervisory assistance is unavailable.
  • Maintains an active caseload, providing referrals and linkages as needed within their area of expertise and credential limits.
  • Gathers and shares detailed information regarding assigned clients to lay the groundwork for the Treatment Plan.
  • Collaborates with clients daily, using professional judgment to implement the team-determined Treatment Plan.
  • Assists in the development, execution, and revision of individual treatment plans, ensuring that services align with the Treatment Plan and monitoring progress toward treatment objectives.
  • Engages with community systems to facilitate linkages, referrals, crisis management, advocacy, and follow-up, focusing on achieving treatment goals.
  • Provides crisis management for clients and facilitates linkages for appropriate interventions.
  • Delivers individual and group Skills Training and Development (STD) sessions.
  • Conducts face-to-face skill training and mental health interventions in accordance with the treatment plan.
  • Teaches age-appropriate life skills training to clients, both individually and in groups.
  • Maintains client and program records in compliance with applicable standards, regulations, and grant requirements.
  • Upholds a high level of ethical conduct regarding confidentiality, dual relationships, and professional integrity.
  • Demonstrates competent use of Essentia.
  • Participates in ongoing education activities to remain knowledgeable in their areas of expertise.
  • Attends relevant meetings and meets regularly with supervisors to exchange pertinent information and receive guidance.
  • Completes required training and certifications timely.
  • Performs additional duties as assigned.

Minimum Qualifications

To qualify for the Senior Care Coordinator position at Hamilton Center, candidates must possess:

  • A Bachelor's degree in relevant fields such as Psychology, Social Work, Sociology, or related disciplines, along with a minimum of 1 year of Case Management experience.
  • Alternatively, candidates with nonspecific Bachelor's degrees must have at least 4 years of experience as a Care Manager in a directly related field.
  • Completion of Case Management training post-degree.
