LPN (Per Diem) - Cape Cod Healthcare

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at VNA of Cape Cod. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our Adult Day Health program.
Key Responsibilities- Implement medical plans of care, nursing interventions, and procedures as necessary for patient care, adhering to licensure guidelines.
- Complete monthly client progress notes and vital signs, ensuring accurate and timely documentation.
- Administer and record medications as ordered by physicians, following established standards of practice.
- Collaborate with the Program Director/RN and Adult Day Health team members to ensure seamless care coordination.
- Assist with feeding, toileting, ambulating, transporting, and other tasks as required, promoting patient comfort and safety.
- Assess and identify changes in clients' conditions and needs, reporting to the Program Director/RN.
- Contribute to the delivery of other required program services, as needed.
- Maintain confidentiality of client/employee information and administrative, marketing, and clinical business practices.
- Communicate essential information to the Program Director/RN to promote high-quality and continuous client care.
- Ensure environmental safety for clients and staff, utilizing equipment and supplies appropriately.
- Follow infection control techniques and consistently function as a positive member of the program team.
- Provide exceptional service to patients, family members, visitors, volunteers, and co-workers, reflecting VNA's commitment to CARES: compassion, accountability, respect, excellence, and service.
- Current LPN license in MA
- CPR certification required
- One year of recent experience in an acute facility
- Experience with older adults preferred
