Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at VNA of Cape Cod. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare organization.

Key Responsibilities
  • Implement medical plans of care, nursing interventions, and procedures as necessary for patient care, adhering to licensure guidelines.
  • Complete monthly client progress notes and vital signs, ensuring accurate and timely documentation.
  • Administer and record medications as ordered by physicians, following established standards of practice.
  • Collaborate with the Program Director/RN and Adult Day Health team members to ensure seamless communication and care coordination.
  • Assist with daily living tasks, such as feeding, toileting, ambulating, and transporting patients, as needed.
  • Monitor and report changes in patient condition and needs to the Program Director/RN.
  • Contribute to the delivery of other required program services, as assigned.
  • Maintain confidentiality of client and employee information, adhering to organizational policies and procedures.
  • Ensure environmental safety for patients and staff, utilizing equipment and supplies effectively and safely.
  • Follow infection control techniques and maintain a clean and safe work environment.
  • Provide exceptional service to patients, family members, visitors, volunteers, and colleagues, embodying VNA's commitment to CARES: compassion, accountability, respect, excellence, and service.
Requirements
  • Current LPN license in Massachusetts
  • CPR certification
  • One year of recent experience in an acute facility
  • Experience working with older adults preferred
