Job Title: RN Case Manager MNA, VNA of Cape CodJob Summary:

We are seeking a skilled RN Case Manager MNA, VNA of Cape Cod to join our team. The successful candidate will be responsible for participating in inter-disciplinary patient case conferences, utilizing critical thinking skills to assess patients and respond to critical changes, and demonstrating excellent communication skills.

Key Responsibilities:
  • Participate in inter-disciplinary patient case conferences as needed.
  • Utilize critical thinking skills to assess patients and respond to critical changes.
  • Utilize organizational skills to manage time efficiently and prioritize patient needs.
  • Demonstrate excellent communication skills, both verbal and written.
  • Interact efficiently and compassionately with patients, families, co-workers, and management.
  • Provide patient care with a compassionate and caring attitude.
  • Notify MD/other involved care providers of significant changes in patient condition.
  • Complete admission and other paperwork on a timely basis.
  • Record narrative documentation to reflect skilled care.
  • Obtain and clarify valid MD orders for all care delivered.
  • Ensure completion of Admission paperwork by 9am of next business day at the latest.
  • Adhere to standards of nursing set forth by the American Nurses Association and demonstrate understanding of hospice philosophy of care.
  • Explain philosophy, purpose, scope, and role of hospice care to patient/family and/or nursing home staff.
  • Implement Plan of Care under direction of attending physician, ensuring compliance with federal and state regulations.
  • Supervise Hospice Aides every 14 days per regulations.
  • Assist in orientation of new staff by acting as a mentor when requested.
  • Assume additional responsibilities as assigned by manager.
Requirements:
  • Current RN license in Massachusetts and other related credentials to meet specialty.
  • One (1) year of nursing experience required or six (6) months of Nursing experience with a BSN.
  • Hospice Home Care experience, IV Certification, Computer skills, and BSN preferred.
