LPN (Per Diem) - VNA of Cape Cod

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at VNA of Cape Cod. As an LPN, you will play a vital role in providing high-quality patient care and collaborating with our interdisciplinary team.
Key Responsibilities- Assess patient needs and report new findings to the RN and MD
- Implement the medical plan of care, nursing interventions, and procedures as necessary
- Evaluate and reassess patient needs on a continuing basis
- Identify ancillary services needed by patients and families and report findings to the RN
- Collaborate with the Team Leader, Office Nurse, RN, and members of the inter-disciplinary team as needed
- Assist patients and families in accessing appropriate community resources
- Record narrative documentation to reflect skilled care
- Graduate of an approved LPN school of nursing
- Current LPN license in MA
- CPR certification required
- Minimum one (1) year recent experience in an acute care facility
- Homecare and computer skills preferred
