Job Summary

The Social Work Manager position at Danbury Hospital is responsible for assisting the Director of Case Management in the day-to-day operations of the Social Workers in the Case Management department. This role requires a strong understanding of social work principles and practices, as well as excellent communication and interpersonal skills.

Key Responsibilities
  • Assist with scheduling of staff and daily assignments to ensure adequate coverage of all units/floors with available budgeted staff.
  • Develop and implement a comprehensive orientation process for new hire Social Workers and evaluate their competency.
  • Serve as a liaison to community post-acute providers and community referral resources.
  • Evaluate Case Management program outcomes and assist with review and analysis of data to identify and address variances in outcomes/performance.
  • Conduct performance evaluations and identify job goals for Social Work staff.
  • Institute performance improvement strategies for Social Work staff experiencing difficulties in performing job-related duties.
  • Participate in the development of department Policies and Procedures.
  • Work collaboratively with the Manager of RN Case Managers to ensure efficient and effective department operations.
  • Maintain contemporary professional knowledge and education.
  • Research and promote the adaptation of best practices to ensure quality patient care and optimal outcomes.
Requirements
  • Masters Degree in Social Work
  • LMSW with current CT license
  • 3-5 years hospital-based social work experience or equivalent experience in a community-based facility (SNF/Community/MH/CPS/APS)
  • Experience in psychosocial and therapeutic counseling
  • Strong interpersonal and communication skills
  • Ability to form positive, collaborative relationships with interdisciplinary staff, patients, and families
  • Ability to negotiate effectively with internal and external providers
  • Ability to manage multiple competing priorities
  • Strong analytical, problem-solving skills
  • Preferred: CCM/ACM-SW certification
