Caregiver Role Summary

We are seeking a Resident Care Coordinator to join our team at Integrinurse Inc. In this role, you will be responsible for coordinating and delivering high-quality care to our residents, working closely with interdisciplinary teams to ensure comprehensive support and services.

Key Responsibilities:
• Coordinate care plans, ensuring that residents receive timely and effective support and services.
• Communicate regularly with residents, families, and healthcare teams to ensure open and transparent care delivery.
• Participate in QAPI processes, identifying areas for improvement and contributing to the development of new policies and procedures.
• Collaborate with colleagues to ensure seamless care coordination and effective communication.
