Home Infusion Coordinator

**Responsibilities**
We are seeking a highly skilled Registered Nurse with at least 1 year of experience in infusion therapy to join our team as a Pharmacy Nurse. As a key member of our nursing staff, you will be responsible for:
- Coordinating infusion therapy services in the home setting
- Collaborating with patients, family members, caregivers, and healthcare professionals to ensure safe transitions
- Documenting patient progress and outcomes in our electronic medical record (EMR)
**Requirements**
To be considered for this role, you must have:
- An Associate's degree in Nursing or equivalent
- A valid New York State Driver's License
- At least 1 year of experience in infusion therapy
**Benefits**
