About the Role
This position involves assessing, planning, implementing, and evaluating patient care plans in the Home Care setting. As a Nursing Care Coordinator, you'll provide leadership to the Healthcare team, fostering a collaborative environment that promotes effective communication and mutual respect. Your expertise will help ensure high-quality patient outcomes and efficient care delivery.

Key Responsibilities
- Coordinate patient care plans in collaboration with healthcare professionals
- Assess, plan, implement, and evaluate patient care in the Home Care setting
- Provide leadership to the Healthcare team, promoting effective communication and collaboration
- Foster a culture of mutual respect and open communication among team members

Requirements
- Associate's Degree or higher in Nursing (RN)
- 1-5 years of relevant experience as a Registered Nurse
- Current license required

Work Environment
- Full-time position
- Days shift with potential evening shifts and rotation of weekends, holidays, and on-call responsibilities
- Salary: $40.80-$50.90 per hour, determined by candidate's experience, qualifications, internal equity, and collective bargaining agreement
