Caregiver for Individuals with Developmental Disabilities

Job Summary


At Constructive Partnerships Unlimited (CP Unlimited), we are seeking a compassionate and dedicated Residential Caregiver to join our team. As a Residential Caregiver, you will play a vital role in ensuring the well-being and care of individuals with developmental disabilities who reside in our residential program.


Key Responsibilities


1. Person-Centered Approach


Utilize a person-centered approach to ensure the well-being and care of individuals with developmental disabilities. This includes performing activities of daily living (ADL) such as bathing, dressing, toileting, grooming, meal preparation, and lifting or transferring activities.


2. Medication Administration


Administer medications and treatments according to doctors' orders and delegated tasks under the clinical supervision of an RN.


3. Individualized Programs


Implement individualized programs and staff action plans in accordance with each person's Life Plan. Maintain records and charts in electronic format as required.


4. Community Activities


Accompany individuals in community activities, medical appointments, and other applicable activities.


5. Housekeeping Duties


Perform general housekeeping duties to maintain the cleanliness and safety of the residence.


Requirements


1. Participate as a member of the interdisciplinary team and assist in evaluating and implementing goals/outcomes.


2. Actively implement individual programs and staff action plans by following prescribed methodology.


3. Regularly communicate and relay pertinent information to clinical team members, supervisors, co-workers, and other appropriate agency personnel.


4. Ensure the well-being and care of individuals by appropriately using specialized adaptive equipment, responding to changes in conditions, participating in fire drills, etc.


5. Participate in recreational activities in the home and accompany individuals to community-based activities.


6. Enter daily data for each person based on their individualized plans (changes in behavior, responses to staff action plans, goal progress, community activity, vital signs, blood sugar readings, bowel regimen, etc.).


7. Maintain recordkeeping practices for each assigned residence in accordance with agency policies.


8. Prepare meals for individuals by following each person's individualized diet and menu plan.


9. Administer medications and/or treatments and document accordingly on each person's Medication Administration Record (MAR).


10. Accompany individuals as needed to their medical appointments.


11. Attend and actively participate in and successfully complete all required training sessions.


12. Know, understand, and follow the DSP Code of Ethics and meet the expectations and standards of the DSP Core Competencies.


13. Perform various miscellaneous duties as assigned, such as shopping for household supplies and going to the pharmacy and laundromat.
