About the Role

We are seeking a compassionate and dedicated Hospice Care Specialist to join our team at Everystep. As a Hospice Care Specialist, you will play a vital role in providing high-quality patient care and support to individuals and their families during a challenging time.

Key Responsibilities
  • Provide Personal Care: Assist patients with activities of daily living, such as bathing, dressing, and grooming, in accordance with a written plan of care and under the direction of the Registered Nurse.
  • Work with the Interdisciplinary Team: Collaborate with the healthcare team to develop and implement patient care plans that focus on quality of life and comfort.
  • Document Patient Care: Accurately document patient care in the electronic health record, ensuring compliance with organizational and professional standards.
  • Provide Emotional Support: Offer emotional support and compassionate care to patients and their families, promoting a sense of comfort and well-being.
  • Participate in Quality Improvement: Actively participate in quality improvement planning and processes to ensure the highest level of patient care.
Requirements
  • Education: High school diploma or equivalent required.
  • Experience: At least 1 year of experience in a nursing home or home health agency preferred.
  • Skills: Strong communication and interpersonal skills, ability to work independently, and flexibility to adapt to changing situations.
  • Certifications: Successful completion of in-house competency testing, mandatory reporter of child and dependent adult abuse training certification, and valid driver's license.
Working Conditions

This role requires the ability to work in a fast-paced environment, with frequent bending, stooping, and lifting. The ideal candidate will be able to maintain a high level of physical and emotional stamina, with a strong commitment to providing exceptional patient care.
