This position involves providing and directing nursing care to patients in their homes, as prescribed by physicians and in compliance with agency policies. Key responsibilities include:

  • Coordinating the total plan of care with other healthcare professionals involved in patient care, ensuring continuity and exchanging information with relevant parties.
  • Making initial and comprehensive nursing evaluations, determining the primary focus of care, developing plans within state-specific guidelines, and submitting accurate documentation in collaboration with physician orders.
  • Performing patient assessments at designated time points and developing appropriate care plans.
  • Ensuring patients meet home health eligibility and medical necessity criteria defined by payer sources.

To be successful in this role, you must possess the following qualifications:

  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
