Nurse Care Specialist

As a Nurse Care Specialist (Registered Nurse) in Home Health, you will play a vital role in providing exceptional patient care and building trusted relationships.

About the Role

This is a part-time opportunity for a Registered Nurse to join our team on a PRN basis. As a Nurse Care Specialist, you will work closely with patients in their homes, delivering comprehensive care that addresses their unique needs. Your primary focus will be to develop and implement personalized plans of care, ensuring seamless continuity of care throughout the patient's journey.

Key Responsibilities

* Conduct initial and comprehensive nursing evaluations, determining the primary focus of care and developing plans within state-specific guidelines
* Coordinate total care plans with other healthcare professionals, exchanging information to ensure effective patient outcomes
* Perform patient assessments at designated time points, developing and updating plans of care as needed
* Ensure patients meet home health eligibility and medical necessity guidelines defined by payer sources

Requirements

* Current RN licensure in your state of practice
* CPR certification
* Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation
