Medical Care Specialist for Seniors

Abode Care Partners is a leading provider of integrated medical services caring for individuals from post hospitalization to home in various settings ranging from skilled nursing facilities, assisted living, independent living, group homes, and private homes.
About the RoleWe are seeking a compassionate Nurse Practitioner (NP) to provide personalized medical care to seniors and patients with complex needs in the Louisville, KY and Southern IN areas.
Responsibilities- Perform evidence-based medical care by planning, directing, and evaluating patient medical needs.
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
- Order and interpret laboratory and diagnostic testing as needed.
- Collaborate with healthcare professionals and caregivers in providing interdisciplinary care and maintaining comprehensive patient records.
- An active state Nurse Practitioners license.
- A current Nurse Practitioner National Board Certification.
- An active state DEA licensure.
- A Master's degree in Nursing (MSN).
- A minimum of five years of nursing experience, with at least two years of nurse practitioner experience preferred.
- Strong interpersonal communication skills - verbal and written.
- Excellent time management skills with accountability for practitioner's daily workflow.
- A competitive salary range of USD $95,000.00 - $130,000.00 per year.
- A flexible schedule with weekdays and daytime shifts only.
