Position Title: Registered Nurse (RN) Case Manager - Hospice

Location: Oakland

Patient Coverage Area: Oakland

Employment Type: Full Time (40 Hours/Week)

Work Schedule: 8:30 AM - 5:00 PM, with rotating weekends and holidays

Compensation: The anticipated pay range is $78.30 per hour, which reflects a reasonable estimate based on the current market conditions. Actual compensation may vary.

Role Summary: The Hospice Nurse is responsible for admitting patients into the service, conducting comprehensive assessments of the patient's and family's needs—physical, psychosocial, environmental, safety, and developmental. This role involves creating a tailored care plan based on assessments and documenting all observations and interventions. The RN collaborates with a multidisciplinary team and participates in quality improvement initiatives while conducting follow-up visits as necessary.

Key Responsibilities:
  • Conduct initial comprehensive assessments of patients.
  • Document clinical findings, observations, and care plans.
  • Collaborate with the Clinical Team Manager to initiate and revise care plans as needed.
  • Evaluate the need for additional services from team members such as PT, OT, ST, MSW, and HHA.
  • Provide skilled nursing care during all patient visits, including teaching patients and families.
  • Obtain and document physician orders accurately.
  • Perform supervisory visits for Home Health Aides in compliance with regulations.
  • Coordinate care with the Clinical Team Manager and other healthcare professionals, ensuring effective communication regarding patient condition changes.
  • Adhere to established standards for documentation and technology use.
  • Submit weekly schedules for patient visits and collaborate on scheduling needs.
  • Conduct re-certifications, transfers, and discharges as directed.
  • Participate in multidisciplinary patient conferences.
  • Demonstrate clinical competencies and engage in ongoing education.
  • Contribute to quality improvement efforts within the agency.
  • Assist in developing agency protocols and educational materials for patients.

Qualifications:
  • Valid California RN license.
  • At least one year of nursing experience is required.
  • Current CPR certification.
  • Valid California driver's license and automobile insurance.
