Job Summary

We are seeking a compassionate and dedicated Social Worker to join our dialysis team at Health Systems Management, Inc. As a Social Worker, you will play a vital role in providing direct and indirect interventions to pre-dialysis and chronic dialysis patients, collaborating with the multidisciplinary healthcare team to assist patients in reaching their fullest rehabilitative potential.

Responsibilities
  • Provide Clinical Services: Conduct comprehensive psychosocial assessments, communicate with patients and their support system to establish a plan of care, and utilize appropriate community resources to meet patient/family concrete needs.
  • Collaborate with Healthcare Team: Work with the multidisciplinary healthcare team to provide clinical services, develop and implement treatment plans, and coordinate patient care.
  • Assess Patient Needs: Conduct thorough assessments of patient needs, identify areas for improvement, and develop strategies to address these needs.
  • Develop and Implement Treatment Plans: Create and implement treatment plans that address patient needs, goals, and outcomes.
  • Communicate with Patients and Families: Communicate effectively with patients and their families to ensure understanding of treatment plans, progress, and outcomes.
Requirements
  • Education: Master's Degree in Social Work required.
  • Licensure: Georgia applicants must be licensed.
  • Skills: Ability to solve practical problems, deal with concrete variables, work independently, and plan/organize priorities autonomously.
  • Experience: Willingness to work a flexible schedule and to fill in when needed.
