We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in a part-time capacity. As an LPN with BrightSpring Health Services, you will have the opportunity to provide high-quality patient care in a home health setting.

Responsibilities

  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Assist with admissions, discharges, and follow-up care as needed.
  • Participate in medical appointments and provide support as required.
  • Deliver nursing services under the direction of a registered nurse (RN) and as ordered by a physician.
  • Provide direct patient care as outlined in the Service Plan/Nursing Care Plan.
  • Monitor and record vital signs, weights, wound measurements, and other essential patient data.
  • Identify trends or concerns and communicate with the RN or physician as necessary.

Requirements

  • Current licensure as a Licensed Practical/Vocational Nurse (LPN or LVN, per state requirements).
  • CPR certification.
  • At least six months of direct nursing experience.
