Job Summary

The Clinical Care Coordinator will be responsible for coordinating patient care across the continuum of care for all patients. This includes educating and providing information to patients, supporting them, and facilitating treatment plans. The ideal candidate will function as a patient advocate, guiding patients and families throughout the cancer care and survivorship experience.

Key Responsibilities

  • Perform intake, assessment, coordination, and navigation of patients.
  • Serve as a liaison for referring physicians seeking information or access to the system.
  • Screen patients for appropriate information and manage the acquisition of pertinent medical records for initial appointments.
  • Plan and implement the patient evaluation process.
  • Facilitate patient evaluation by interfacing with referring physicians and MGUH services.
  • Coordinate complex patient management at various stages of diagnosis, treatment, and follow-up, collaborating with all disciplines involved in patient care.
  • Navigate patients through the continuum of care, providing education and resources about medical conditions, diagnostic procedures, treatments, clinical trials, support services, hospital processes, and other relevant information.
  • Communicate patient progress and disposition to healthcare providers.
  • Aid in maintaining outcome databases.
  • Have a working knowledge and access to multiple electronic systems to facilitate patient care and documentation.
  • Identify needs and develop patient education and support materials.
  • Provide community education and outreach.
  • Demonstrate competencies, including knowledge and behaviors, to meet specific patient care requirements, nursing practice standards, and other regulatory or required knowledge and/or skills.

Requirements

  • Bachelor's degree in Nursing required.
  • Master's degree in Nursing preferred.
  • 3-4 years of clinical nursing experience required.

  • Clinical maturity and experience.
  • Excellent interpersonal skills.
  • Ability to work collaboratively with an interdisciplinary team of healthcare providers.
  • Effective oral and written communication skills.
  • High level of advocacy and problem-solving ability.
  • Ability to multitask and organize advocacy and support for a large and varied group of cancer patients and their families.
