Position Summary:
The Clinical Nurse Specialist is responsible for delivering comprehensive nursing care while ensuring patient safety and promoting optimal health outcomes. This role is integral to the interdisciplinary healthcare team, collaborating effectively to provide exceptional patient care.

Qualifications:
  • Experience: Minimum of 12 months of nursing experience required.
  • Preferred Qualifications: Bachelor of Science in Nursing and at least 1 year of clinical experience.
  • Education: Graduation from an accredited nursing program.
  • Licensure: Active Registered Nurse license in Georgia or Multi-State through the Enhanced Nurse Licensure Compact.
  • Certifications: Basic Life Support (BLS) certification within 30 days of employment; Pediatric Advanced Life Support (PALS) or Pediatric Emergency Assessment Recognition and Stabilization (PEARS) within one year of employment as required by the department.

Core Responsibilities:
  • Patient Care: Deliver direct patient care while ensuring effective communication with the healthcare team to facilitate coordinated care.
  • Collaboration: Work closely with home health agencies, third-party payers, and community resources to address patient needs.
  • Care Coordination: Oversee outpatient and clinic care coordination, monitoring patient progress between clinic visits and home care.
  • Telehealth Services: Provide care through telehealth platforms to address patient concerns and needs.

Professional Development:
  • Engage in continuous learning to enhance nursing practice and maintain professional competence.
  • Incorporate new knowledge into practice to achieve desired patient outcomes.
  • Maintain accurate professional records to demonstrate ongoing learning and competence.

Leadership and Engagement:
  • Supervise nursing care provided by unlicensed personnel while ensuring accountability for care quality.
  • Foster a supportive work environment and contribute to a culture of safety.
  • Participate in quality improvement initiatives to enhance patient outcomes and operational efficiency.

Mandatory Requirements:
  • Complete orientation and competency assessments by the designated deadlines.
  • Fulfill mandatory education and annual competency requirements as specified.
  • Maintain a minimum attendance at staff meetings as applicable.
