Job Summary:
Care for young adults and youth between the ages of 12 to 30 by providing intensive wraparound services, including relationship building, coaching/mentoring, safety planning, strength and need assessments, goal planning, coordination of community services, and systems navigation and advocacy.

The Community Passageways team is dedicated to making a positive impact in our community. We're seeking an experienced Resource Navigator to join our team and work with us to create meaningful change. As a key member of our organization, you will be responsible for navigating our participants through complex systems, providing guidance and support every step of the way.

Responsibilities:
- Connect with young adults and youth who meet specific risk factors, establish a supportive relationship, and maintain consistent contact via phone calls, texts, and in-person meetings
- Review service plans with participants every 60 days, addressing barriers and celebrating accomplishments
- Provide referrals to comprehensive wrap-around services such as housing, employment, medical support, mental health, substance use, legal advocacy, etc.
- Develop and maintain relationships with systems players, act as an advocate for participants when dealing with other agencies, and support participants to develop their voice within systems
- Maintain clear, professional participant contact notes, service plans, progress updates, and documentation according to all confidentiality guidelines
- Attend weekly supervision meetings to review workload, highlight successes, and find solutions to challenges

Salary Estimate:
$63,000 - $80,000 per year (dependent on location and experience)
