Position Overview
The Medication Therapy Management (MTM) Pharmacist plays a crucial role in evaluating the prescription, medical, and laboratory claims history of targeted patients. This position aims to uncover opportunities for enhancing therapeutic outcomes and minimizing the risk of adverse events. The MTM Pharmacist engages with identified patients to offer education and coaching, as well as to explore potential improvements in patient care. Furthermore, collaboration with prescribers is essential to facilitate necessary adjustments to medication regimens.
Qualifications
- A Bachelor's degree is required; a PharmD degree is preferred. Must be licensed to practice pharmacy in New York.
- A minimum of five (5) years of experience in providing pharmaceutical care services is required, with preference given to those with MTM experience.
- Strong verbal and written communication skills, with a proven ability to interact effectively with other healthcare professionals.
- In-depth knowledge and skills to conduct comprehensive drug regimen reviews, supported by a robust clinical knowledge base maintained through ongoing education and professional affiliations.

Key Responsibilities
- Exhibits a thorough understanding of clinical protocols, policies, and procedures necessary for conducting drug regimen reviews.
- Provides education to members over the phone, actively listening for opportunities to enhance drug regimens.
- Collaborates with prescribers to implement appropriate modifications in drug regimens.
Immigration or work visa sponsorship is not available for this position.
Compensation Range: $115,000 - $130,000 annually, with variations based on skills, education, location, and experience.
In addition to base compensation, associates may qualify for scorecard incentives, a comprehensive benefits package, and generous paid time off. The base salary range is subject to change.
