Position Overview

OHI is transitioning to a four-day workweek with ten-hour shifts. Candidates must be prepared to adapt to this schedule.

Role Summary

The Clinical Quality Outreach Navigator will report directly to the Director of Nursing and will be responsible for spearheading quality enhancement initiatives for various performance metrics as designated by Ocean Health Initiatives (OHI). This role involves direct patient interaction, including outreach via phone, meticulous documentation in electronic medical records (EMR), and executing elements of chronic care management.

Key Responsibilities

  • Conduct proactive outreach to patients who may benefit from chronic care management services.
  • Perform follow-up outreach to track lab results, imaging, referrals, and other quality improvement measures.
  • Engage patients in the development, execution, and monitoring of personalized care plans aimed at achieving optimal health outcomes.
  • Utilize effective communication strategies to interact compassionately with patients and their support systems, leveraging Social Determinants of Health data to tailor individualized care plans.
  • Assist patients in completing intake forms to identify their needs and resource gaps that may hinder goal attainment.
  • Maintain regular telephonic communication with patients and their significant others to address care gaps.
  • Collaborate with other healthcare team members, including providers and consultants, to ensure comprehensive care and accurate EMR updates.
  • Prepare and submit all necessary Care Delivery Intervention Reports to the Director of Nursing.
  • Act as a liaison among patients, primary care practices, and quality improvement teams to facilitate effective care delivery.
  • Establish partnerships with community organizations to support patients in overcoming barriers to care.
  • Employ evidence-based coaching techniques to assist patients in setting realistic health objectives and implementing positive lifestyle changes.
  • Contribute as a member of the interdisciplinary team to fulfill the objectives of the practice, providers, and patients, while supporting population health initiatives.
  • Deliver exceptional customer service while adhering to HIPAA regulations.
  • Document patient interactions and care planning activities in the EMR as directed by the care team.
  • Identify and escalate patient-related issues to the provider as necessary.
  • Utilize data analytics to tailor interventions for specific patient populations.
  • Participate in meetings to provide updates on patient status and care initiatives.
  • Support transformation efforts as required.
  • Travel regionally using a personal vehicle, with mileage reimbursement in accordance with company policy.
  • Perform additional duties and projects as assigned.

Additional Clinical Duties May Include

  • Administer nursing care, including medication administration and immunizations, as directed by providers.
  • Conduct patient intake assessments by documenting medical history, current medications, and relevant social history.
  • Measure and record vital signs, ensuring accurate documentation in the EMR.
  • Assist in patient education regarding health maintenance and lifestyle choices.
  • Support providers in managing chronic conditions through education and disease management strategies.
  • Maintain compliance with safety and infection control protocols.
  • Ensure proper management of medical supplies and maintain accurate logs for inventory.
  • Document and report significant patient conditions and reactions to treatments.
  • Engage in ongoing professional development and training to enhance skills and knowledge.

Qualifications

  • Graduation from an accredited Certified Medical Assistant or Licensed Practical Nursing program is required.
  • One to three years of relevant experience in medical or community health settings is necessary.
  • Current certification as a CMA or active LPN license in the State of New Jersey is required.
  • Basic Life Support certification from the American Heart Association is mandatory.
  • Experience in teaching or training within a healthcare context is preferred.
  • Proficiency in Microsoft Office 365 is essential.
  • Bilingual capabilities in English and Spanish are advantageous.
  • Flexibility to accommodate varying work hours, including evenings and weekends, is necessary.
  • Reliable transportation is required for travel within designated regions.
