Position Overview

Ocean Health Initiatives is seeking a dedicated Clinical Quality Outreach Navigator to enhance our quality improvement initiatives. This role is pivotal in coordinating efforts related to various health measures and indicators.

Key Responsibilities

  • Conduct proactive outreach to patients who may benefit from chronic care management services.
  • Perform follow-up communications for lab/image/referral tracking and address gaps in care.
  • Engage patients in the development and execution of personalized care plans aimed at achieving optimal health outcomes.
  • Utilize effective communication strategies to collaborate with patients and their support systems, leveraging Social Determinants of Health to tailor care plans.
  • Assist patients in completing intake documents to identify their needs and facilitate goal attainment.
  • Maintain regular telephonic contact with patients and their significant others to ensure continuity of care.
  • Coordinate with the interdisciplinary care team and providers to address care gaps and update electronic medical records (EMR).
  • Submit all Care Delivery Intervention Reports to the Director of Nursing, ensuring compliance with relevant agreements.
  • Act as a liaison among patients, primary care practices, and quality teams to streamline care delivery.
  • Foster partnerships with community resources to support patients in overcoming barriers to care.
  • Employ evidence-based coaching techniques to assist patients in setting realistic health goals and implementing lifestyle changes.
  • Collaborate with the care team to achieve the objectives of the practice and support population health initiatives.
  • Provide exceptional customer service while adhering to HIPAA regulations.
  • Document patient interactions and care planning activities in the EMR as directed by the care team.
  • Identify and escalate patient-related issues to providers as necessary.
  • Utilize data analytics to tailor interventions for specific patient populations.
  • Participate in meetings to present status updates and support transformation efforts.
  • Travel regionally as required, using personal transportation with mileage reimbursement.
  • Perform additional duties and projects as assigned.

Additional Clinical Duties May Include

  • Administer nursing care, including medication administration and patient education on health management.
  • Document patient intake information, including medical history and vital signs, in the EMR.
  • Assist providers in educating patients on healthy lifestyle choices and disease management.
  • Monitor patient flow and prepare examination areas for patient visits.
  • Ensure compliance with safety and infection control protocols.
  • Maintain accurate logs for medical supplies and referrals.
  • Participate in ongoing professional development and training.

Qualifications

  • Graduation from an accredited Certified Medical Assistant or Licensed Practical Nursing program.
  • One to three years of relevant experience in healthcare settings.
  • Active certification or licensure in the state of New Jersey.
  • Basic Life Support certification from an accredited organization.
  • Proficiency in Microsoft Office 365.
  • Bilingual capabilities in English and Spanish are preferred.
  • Flexibility to meet varying work hours and cover shifts as needed.
