The Clinical Quality Outreach Navigator plays a crucial role in enhancing healthcare quality by overseeing improvement initiatives related to various performance metrics identified by Ocean Health Initiatives (OHI). This position involves direct patient interaction, including telephonic outreach, meticulous documentation in electronic medical records (EMR), and the execution of chronic care management tasks.

Key Responsibilities:

  • Conduct proactive outreach to patients who may benefit from chronic care management services.
  • Perform follow-up communications to ensure tracking of laboratory results, imaging, referrals, and address any gaps in care.
  • Collaborate with patients to develop, implement, and monitor personalized care plans aimed at achieving positive health outcomes.
  • Utilize effective communication strategies to engage patients and their support systems, leveraging Social Determinants of Health data to inform individualized care plans.
  • Assist patients in completing intake forms to assess their needs and identify knowledge, skills, and resource gaps.
  • Engage in frequent telephonic interactions with patients and their significant others to address care gaps.
  • Coordinate with interdisciplinary team members, including providers and consultants, to ensure comprehensive care and accurate EMR updates.
  • Prepare and submit Care Delivery Intervention Reports to the Director of Nursing, adhering to established protocols.
  • Act as a liaison among patients, primary care practices, and quality improvement teams to facilitate effective care delivery.
  • Establish partnerships with community resources to assist patients in overcoming barriers to care.
  • Employ evidence-based coaching techniques to empower patients in setting realistic health goals and making positive lifestyle changes.
  • Participate as a member of the interdisciplinary team to support the objectives of the practice and enhance patient care.
  • Deliver exceptional customer service while maintaining compliance with HIPAA regulations.
  • Document patient interactions and care planning activities in the EMR as directed by the care team.
  • Identify and escalate patient-related issues to the provider as necessary.
  • Utilize data analytics to tailor interventions for specific patient populations.
  • Attend and present updates at team meetings.
  • Support transformation initiatives as required.
  • Travel regionally as needed, with personal vehicle use reimbursed according to company policy.
  • Perform additional duties and projects as assigned.

Additional Responsibilities:

  • Administer nursing care, including medication administration and patient education on health management.
  • Document vital signs and other pertinent health information in the EMR.
  • Assist providers in educating patients on lifestyle modifications and chronic disease management.
  • Maintain compliance with all health and safety protocols.
  • Manage inventory of medical supplies and ensure proper documentation of referrals.
  • Engage in ongoing professional development and training to enhance skills and knowledge.

Qualifications:

  • Graduate of an accredited Certified Medical Assistant or Licensed Practical Nursing program.
  • One to three years of relevant experience in medical or community health settings.
  • Active certification or licensure in the state of New Jersey.
  • Basic Life Support certification from the American Heart Association.
  • Proficiency in Microsoft Office 365; bilingual skills are a plus.
  • Flexibility to accommodate practice hours, including evenings and weekends.
  • Reliable transportation for regional travel.
