JOB SUMMARY:


In partnership with physicians, registered nurses, and other healthcare professionals, the Nurse Practitioner delivers comprehensive, empathetic care to patients and their families while managing care coordination and management activities on behalf of the PruittHealth Premier Plan.

This role involves collaborative management with physicians to diagnose, treat, and prevent both acute and chronic health conditions, as well as promoting overall wellness.

Nurse Practitioners operate with advanced nursing practices that encompass independent nursing functions aligned with established nursing care standards, while also engaging in medical management under a collaborative agreement with physicians.


KEY RESPONSIBILITIES:
- Demonstrates proficiency in conducting thorough physical examinations for adults and the elderly.
- Identifies, diagnoses, and documents chronic health issues in adult patients.
- Recognizes conditions and behaviors that may pose risks to senior patients.
- Actively seeks feedback and engages in ongoing professional development.
- Effectively manages medication reconciliation.
- Identifies and documents the needs of patients with complex clinical profiles.
- Collaborates with physicians, case managers, social workers, family members, and other relevant healthcare personnel.
- Participates in and/or leads Quality Assurance Performance Improvement (QAPI) initiatives.
- Understands and communicates the benefits and policies of the PruittHealth Premier Plan to providers and members.
- Assesses the medical necessity of ancillary services and communicates effectively with providers and team members.
- Coordinates patient care as they transition through various levels of care, ensuring timely communication of the Individual Care Plan (ICP).
- Develops a visit schedule based on approved nurse practitioner workflows.
- Ensures all members have an ICP established within 48 hours of the Health Risk Assessment (HRA).
- Facilitates the implementation of the member's care plan by engaging beneficiaries, facility staff, families, and physicians.
- Evaluates the effectiveness and efficiency of care plans, making adjustments as necessary.
- Utilizes established practice guidelines and protocols from the PruittHealth Premier Plan.

MINIMUM EDUCATION REQUIRED:
Graduate of an accredited Master's Degree program for Adult or Family Nurse Practitioners.

MINIMUM EXPERIENCE REQUIRED:
At least 2 years of clinical experience as a Nurse Practitioner in a geriatric, adult, or family practice setting, with demonstrated ability to conduct comprehensive physical examinations.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current and unrestricted RN and NP licensure in the state of practice. Board certification by AANP or ANCC in Adult, Family Practice, and Geriatrics or Adult-Gerontology Primary Care. Current CPR certification.

ADDITIONAL QUALIFICATIONS:

Graduate of an accredited Geriatric, Adult, or Family Nurse Practitioner Master's Degree program. Previous experience in long-term care is strongly preferred.

KNOWLEDGE, SKILLS, AND ABILITIES:
- Relevant clinical experience pertaining to the population served.
- Ability to review and interpret medical and social histories of members.
- Competence in interpreting laboratory results and recommending follow-up actions.
- Effectively manages both medical and behavioral chronic and acute conditions in collaboration with physicians or specialty providers.
- Synthesizes information to accurately diagnose and develop comprehensive care plans.
- Ensures that individual care plans are tailored to meet the needs identified in the Health Risk Assessment.
- Conducts comprehensive assessments and documents findings in compliance with documentation requirements and CMS regulations.
- Performs Health Risk Assessments within specified timeframes for new beneficiaries and annually thereafter.
- Conducts annual Comprehensive Geriatric Exams that include CMS Annual Wellness Visit elements.
- Ensures accurate ICD-10 coding for all diagnoses documented during visits.
- Reviews treatment orders and interventions for appropriateness and effectiveness.
- Evaluates care plans for cost-effectiveness while addressing the needs of members, families, and providers.
- May participate in on-call programs; after-hours coverage may be required.
- Rotating on-call responsibilities may apply.
- Travel between care sites is mandatory.
- Performs other duties as assigned.
