Position Overview

CommonSpirit Health, a collaboration between Catholic Health Initiatives and Dignity Health, encompasses a network of 275 healthcare professionals dedicated to delivering exceptional care across various locations in central and eastern Kentucky. Our comprehensive service network offers a wide range of Primary Care Services, including Family, Internal, Geriatric, and Pediatric Medicine, alongside multiple specialties tailored to meet the distinct needs of our patients.

Key Responsibilities
  • Conduct thorough assessments of patient conditions and maintain accurate documentation to facilitate communication with fellow providers and support billing processes.
  • Perform detailed evaluations of patients, encompassing functional, mental, social, and physical health aspects, leading to precise diagnoses.
  • Consult with supervising physicians as necessary to validate assessments and ensure optimal patient care.
  • Engage patients and their families in the development of treatment plans, fostering a collaborative approach to healthcare.
  • Act as a liaison between the clinic and other healthcare providers to guarantee timely and comprehensive patient services.
  • Independently create and execute treatment strategies while monitoring patient outcomes effectively.
  • Ensure clear communication of patient requirements, care plans, and resolutions to physicians and interdisciplinary teams.
  • Possess the expertise to supervise clinical staff as required.
  • Formulate and uphold policies and procedures governing patient care activities.
  • Utilize process improvement methodologies to enhance team performance and contribute to program development.
  • Actively engage in departmental and hospital initiatives and committees as deemed appropriate.
  • Provide clinical assistance to physicians operating in satellite clinics.
Qualifications

Essential Qualifications:

  • Valid APRN or PA-C licensure in Kentucky.
  • Eligibility for a Medicare provider number and DEA registration.

Desirable Qualifications:

  • Prior experience in patient care as an APRN or PA-C is preferred.
