Position Overview

Our comprehensive care model includes a wide range of Primary Care Services alongside various specialties tailored to meet the distinct needs of our patients.

Key Responsibilities
  • Conduct thorough assessments of patient health and maintain accurate documentation to facilitate effective communication with the healthcare team.
  • Perform comprehensive evaluations of patients, addressing functional, mental, social, and physical health aspects to establish precise diagnoses.
  • Collaborate with physicians for consultations and supervision as necessary to ensure accurate patient assessments.
  • Engage patients and their families in the treatment planning process, ensuring their involvement and understanding.
  • Act as a liaison between the clinic and other healthcare providers to guarantee timely and comprehensive patient services.
  • Independently create and execute treatment plans while monitoring and evaluating patient outcomes.
  • Communicate patient needs, care strategies, and resolutions to physicians and other members of the healthcare team effectively.
  • Provide guidance and supervision to clinical staff as required.
  • Develop and uphold policies and procedures for patient care operations.
  • Utilize process improvement methodologies to enhance team performance and program development.
  • Actively engage in departmental projects and committees as appropriate.
  • Offer clinical assistance to physicians in various care settings.
Qualifications

Minimum Requirements:

  • Valid APRN or PA-C licensure in Kentucky.
  • Eligibility for a Medicare provider number and DEA registration.

Preferred Qualifications:

  • Prior experience in APRN/PA-C patient care is advantageous.
