Key Responsibilities- Conduct thorough assessments of patient health, documenting findings accurately to facilitate effective communication with the healthcare team.
- Perform comprehensive evaluations encompassing functional, mental, social, and physical health aspects, leading to precise diagnoses.
- Collaborate with physicians for consultations to validate assessments when necessary.
- Engage patients and their families in the development of treatment plans, ensuring their involvement in the care process.
- Act as a liaison between the clinic and other healthcare providers to guarantee timely and comprehensive patient services.
- Independently formulate and execute treatment strategies while monitoring patient progress and outcomes.
- Communicate patient requirements, care plans, and resolutions to physicians and healthcare teams effectively.
- Provide guidance and oversight to clinical staff as required.
- Establish and uphold policies and procedures for patient care operations.
- Utilize process improvement methodologies to enhance team performance and program development.
- Participate actively in departmental projects and committees as appropriate.
- Offer clinical assistance to physicians in various practice settings.
Minimum Requirements:
- Current licensure as an APRN or PA-C in Kentucky.
- Eligibility for Medicare provider number and DEA registration.
Preferred Qualifications:
- Prior experience in patient care as an APRN or PA-C is preferred.
