Job Overview

SUMMARY:

The Native American Community is dedicated to enhancing the overall health and well-being of Native American families. We provide a comprehensive suite of healthcare services, including Medical, Behavioral Health, Dental, and Substance Abuse support. A significant portion of this role will involve assisting patients in navigating social challenges such as health insurance, transportation, and housing.

KEY RESPONSIBILITIES:

  • Execute care coordination tasks as assigned by the Patient Advocate Supervisor, which includes identifying resources and collaborating with patients to overcome obstacles that hinder their access to medical, dental, and behavioral health treatment plans.
  • Serve as a social resource expert within the organization, aiding providers and staff in addressing patients' social requirements.
  • Participate in outreach initiatives to promote the organization within the community; attend health fairs, events, and expos to disseminate information, advocate for services, and enhance community health.
  • Assist uninsured patients in enrolling for medical insurance and maintain current knowledge of community resources.
  • Engage with patients in the clinic to evaluate and assist with their social needs, conducting both in-reach and outreach to connect them with social services.
  • Act as a patient advocate, striving to address community health needs.
  • Perform additional duties as assigned by management.
  • Provide transportation for patients to and from appointments as necessary using the clinic vehicle (no commercial driver's license required).
  • Communicate effectively with patients, registration staff, clinic personnel, and colleagues to ensure safe transportation to appointments.
  • Coordinate transportation activities, including pick-ups, returns, will-calls, discharges, and transfers.
  • Understand the organization's transportation policies and possess knowledge of patient needs, limitations, and available transportation options.

QUALIFICATIONS:

  • Associate's degree (A.A.) or equivalent from a two-year college or technical school; or six months to one year of relevant experience and/or training; or an equivalent combination of education and experience.
  • Proficient in computer skills, including Internet Software, Spreadsheet Software (Excel), Word Processing Software (Word), and Electronic Mail Software (Outlook).
  • Possess an active driver's license and maintain a clean driving record.
  • Experience driving a commercial vehicle is preferred.
  • Previous work experience in a non-profit and healthcare setting is advantageous.
  • Ability to work effectively in a culturally diverse environment is preferred.
  • Experience working within the Native American community is a plus.
