Position Overview:

We are seeking a detail-oriented and dedicated professional to join our team as a Patient Care Coordinator. The successful candidate will have a passion for providing a high level of customer service to home care patients and will be responsible for ensuring a seamless patient experience.

Patient Care Coordinator Responsibilities:

  • Send and track required face-to-face documentation for Physicians to sign and return
  • Collect information for currently hospitalized clients: H&P (History & Physical), ROC (Resumption of Care), MD orders, etc.
  • Conduct pre-admit calls to verify client information, as well as five-day and discharge follow-up calls
  • Track hospitalized clients and notify Community Liaisons of discharges, in order to resume providing care

Patient Care Coordinator Qualifications:

  • High school graduate; college degree preferred
  • Knowledge of medical terminology is strongly preferred
  • Minimum of one (1) year of experience in an office or healthcare setting preferred
  • Strong verbal, written, and telephonic communication skills
  • Demonstrated ability to type at least 50 words per minute
