As a Clinical Care Program Coordinator, your responsibilities will include:

  • Establishing and nurturing relationships with key healthcare facilities, including hospitals and nursing homes, to provide ongoing insights into our services.
  • Conducting facility visits at the request of physicians to assist the clinical team in assessing patient eligibility.
  • Facilitating effective communication and collaboration among program staff and field sales teams through regular meetings. Actively participating in business development discussions, presenting relevant data and updates on accounts and referral sources.
  • Supporting the program in the prompt processing of physician orders as needed.

Essential Qualifications/Skills:

  • Valid RN/LPN/LVN license.
  • In-depth knowledge of healthcare regulations, Medicare eligibility criteria, and a comprehensive understanding of patient care planning and resource coordination.
  • Exceptional customer service, account management skills, organizational abilities, time management, problem-solving, communication, and sales expertise.

Preferred Qualifications/Skills:

  • Bachelor's degree in Nursing is preferred.
  • A minimum of three years of clinical experience is preferred.

Compensation Range

The salary range below reflects a good faith estimate of starting base pay for full-time employment. The actual pay may vary based on geographic location and individual qualifications, including skills, knowledge, experience, education, and certifications. $57,700 - $79,500 per year
