About the Role

As a Patient Care Coordinator at CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes. This position requires strong communication and organizational skills, as well as the ability to work effectively with healthcare professionals and patients.

Key Responsibilities
  • Develop and maintain relationships with healthcare providers, hospitals, and other healthcare organizations to ensure seamless care coordination.
  • Conduct facility visits to assess patient needs and develop personalized care plans.
  • Collaborate with program staff and field sales resources to ensure effective communication and coordination of care.
  • Process physician orders in a timely manner and ensure accurate documentation.
  • Participate in weekly business development meetings to discuss account updates, referral sources, and other relevant information.
Requirements
  • RN/LPN/LVN license required.
  • Strong knowledge of governmental regulations and Medicare eligibility requirements.
  • Comprehensive understanding of potential care plan needs for patients and coordination of necessary resources.
  • Excellent customer service, account development, and communication skills.
Preferred Qualifications
  • Bachelor's of Science in Nursing preferred.
  • Minimum of three years clinical experience preferred.
