As a Patient Care Coordinator at CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes. Key Responsibilities- Develop and maintain relationships with healthcare providers, hospitals, and other healthcare organizations to ensure seamless care coordination.
- Conduct facility visits to assess patient needs and develop personalized care plans.
- Collaborate with program staff and field sales resources to ensure effective communication and coordination of care.
- Process physician orders in a timely manner and ensure accurate documentation.
- Participate in weekly business development meetings to discuss account updates, referral sources, and other relevant information.
- RN/LPN/LVN license required.
- Strong knowledge of governmental regulations and Medicare eligibility requirements.
- Comprehensive understanding of potential care plan needs for patients and coordination of necessary resources.
- Excellent customer service, account development, and communication skills.
- Bachelor's of Science in Nursing preferred.
- Minimum of three years clinical experience preferred.
CenterWell Home Health specializes in personalized, comprehensive home care for patients managing a chronic condition or recovering from injury, illness, surgery, or hospitalization. Our care teams include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers – all working together to help patients rehabilitate, recover, and regain their independence.
