As a Patient Care Coordination Specialist, your responsibilities will include:
- Establishing and nurturing relationships with key healthcare facilities, including hospitals and nursing homes, to provide continuous updates on available services.
- Conducting facility visits at the request of physicians to assist the clinical team in assessing patient eligibility.
- Facilitating effective communication and collaboration among program staff and field sales teams through regular meetings. Actively participating in weekly business development discussions, presenting relevant data and updates regarding accounts and referral sources.
- Supporting the program in the timely processing of physician orders as needed.
Utilize your expertise to create a positive impact
Essential Qualifications:
- Current RN/LPN/LVN licensure.
- In-depth knowledge of government regulations, Medicare eligibility, and a comprehensive understanding of patient care plans and resource coordination.
- Exceptional customer service skills, account development experience, organizational abilities, time management, problem-solving skills, and effective communication.
Preferred Qualifications:
- Bachelor's degree in Nursing is preferred.
- A minimum of three years of clinical experience is preferred.

The salary range provided reflects a good faith estimate of starting base pay for full-time employment. The actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications. $57,700 - $79,500 per yearBenefits Overview:
