Job Title: Child Welfare Case Manager

Lutheran Services Florida (LSF) is seeking a dedicated and compassionate Child Welfare Case Manager to join our team. As a key member of our organization, you will play a vital role in providing full wrap-around case management services to our clients and families.

Job Summary:

The Child Welfare Case Manager will be responsible for accepting cases assigned by the unit supervisor, conducting home visits, developing and implementing case plans, and ensuring the safety and well-being of children in care. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations.

Key Responsibilities:
  • Accept cases assigned by the unit supervisor and conduct home visits to assess the needs of clients and families.
  • Develop and implement case plans that address the identified strengths and needs of clients and families.
  • Ensure the safety and well-being of children in care by conducting regular home visits and monitoring their progress.
  • Collaborate with clients, family members, and other stakeholders to develop and implement effective case plans.
  • Document all case activities and progress in the Florida Safe Families Network database.
  • Provide referrals to community resources and services as needed.
  • Attend court hearings and participate in case staffing as required.
Requirements:
  • Bachelor's degree in a Human Services field.
  • Minimum of one year of relevant experience in child welfare or a related field.
  • Certification in child welfare case management or the ability to become certified within one year.
  • Valid Florida driver's license and insurance.
  • Ability to travel locally and out of the area as needed.
