Job Title: Child Welfare Case Manager

Lutheran Services Florida (LSF) is seeking a dedicated and compassionate Child Welfare Case Manager to join our team. As a key member of our organization, you will play a vital role in providing full wrap-around case management services to our clients and families, while establishing a relationship with them and living the agency's core values.

Job Summary:

The Child Welfare Case Manager will be responsible for providing case management services to children and families, including conducting home visits, developing and implementing case plans, and coordinating with community resources to ensure the best possible outcomes for our clients.

Key Responsibilities:
  • Conduct home visits and assessments to identify the needs of children and families
  • Develop and implement case plans that address the identified needs and goals of children and families
  • Coordinate with community resources, such as healthcare providers, educational institutions, and social services agencies, to ensure the best possible outcomes for our clients
  • Provide support and guidance to children and families to help them achieve their goals and overcome challenges
  • Document all case activities and progress in a timely and accurate manner
  • Collaborate with other professionals, such as social workers, therapists, and educators, to ensure a comprehensive and coordinated approach to case management
Requirements:
  • Bachelor's degree in a human services field, such as social work, psychology, or education
  • Minimum of one year of relevant experience in case management or a related field
  • Valid Florida driver's license and reliable transportation
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to work with diverse populations, including children and families from various cultural and socioeconomic backgrounds
