The LTSS Service Coordinator is a critical role within our organization, responsible for providing exceptional support to individuals with specialized needs. As a key member of our team, you will play a vital role in ensuring seamless care coordination and management of physical health, behavioral health, and long-term services and supports needs.
Key Responsibilities:- Conduct face-to-face program assessments to identify individual needs and develop comprehensive care plans.
- Collaborate with individuals, families, and healthcare teams to ensure cost-effective and efficient utilization of long-term services and supports.
- Identify members with potential clinical health care needs and coordinate cases with clinical healthcare management and interdisciplinary teams.
- Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of long-term services and supports.
- Document short and long-term service and support goals in collaboration with individuals and their chosen care teams.
- Identify members who may benefit from alternative levels of service or other waiver programs.
- Serve as a mentor, subject matter expert, or preceptor for new staff, assisting in the formal training of associates and participating in process improvement initiatives.
- Bachelor's degree in a healthcare-related field or equivalent experience.
- Minimum of 2 years of experience working with a social work agency or equivalent experience.
- Strong understanding of person-centered planning, assessment, and care coordination.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
